Immunization Clinics Consent Form

Ensure Your Rising 7th Grader is Prepared: Take Advantage of Free In-School Immunization Clinics

PGCPS will host free, in-school immunization clinics in June to help current 6th grade students meet the state vaccine requirements for 7th grade.

Vaccines Offered:

  • Tdap
  • Meningococcal (MCV4)

Both vaccines are required for 7th grade entry to prevent exclusion from school starting September 15.

There is no cost to families.

What you need to do:

  1. Download and complete:
  2. Return the signed form to your school nurse by June 2. Forms can be returned in-person or emailed to your school nurse.
